1. You must be a member of an Arizona Fraternal Order of Police Local Lodge.
2. Print the Application Form, which is in Adobe PDF format, and the ACH form. If you need a free Adobe viewer installed on your computer, click here
3. Fill out the application. Attach a check payable to FOPALC for the first month's dues of $20 and the completed ACH authorization.
4. Give the form to your Lodge Secretary or ALC representative.
5. You will be mailed your ALC member packet as soon as the application is processed.
